Filling Out the Death Certificate

NAME OF DECEASED
| This line on the Death Certificate is used to
identify the decedent.
Enter first, middle, and last names of the decedent. Also enter maiden name when applicable. Do not abbreviate. Alias or “also known as” names should also be entered above the legal
name or in parentheses e.g., (AKA-Smith). If decedent had no middle name,
leave blank; do not put “NMI” or “NMN."
The processing of the death certificate typically begins with a person being pronounced dead. It sometimes happens that the name of the deceased is not known at the time of pronouncement of the the death (in the emergency room, for example, with a DOA with no identification). Hospitals generally have specific procedures for temporarily identifying
the deceased -- sometimes, for example, the name will be penciled in, on
the margin of the death certificate so that the form can be processed.
